Losing Weight but Feeling Tired? Here's Why

Fatigue during weight loss — particularly in the first few weeks on GLP-1 medications like Mounjaro — is one of the most common concerns patients raise. It's not a sign that something is wrong with the treatment. It's usually a sign that your body is adapting to a new energy state, and in most cases it's addressable.

The Most Common Causes of Fatigue During Weight Loss

Reduced calorie intake

GLP-1 medications work partly by reducing appetite. Eating significantly less means less energy coming in. If the deficit is large, fatigue follows. The answer is protein-first eating, not eating less.

Iron deficiency

Reduced appetite means less iron from food. Iron deficiency anaemia is a major cause of fatigue during active weight loss — and is easily missed if no one checks. A ferritin blood test gives the answer.

Dehydration

Nausea from GLP-1 medications can reduce fluid intake. Losing glycogen (from reduced carbohydrate intake) also releases water — early weight loss includes fluid. Both combine to cause dehydration-related fatigue.

Electrolyte shifts

Sodium, potassium, and magnesium can shift during rapid early weight loss. Low magnesium particularly causes fatigue and muscle cramping. Dietary variety and adequate fluid intake usually prevent this.

Rapid glucose drop

GLP-1 medications lower blood glucose. If you were previously running consistently high, moving to normal glucose levels can briefly cause "low blood sugar" symptoms even at normal glucose values.

Poor sleep quality

Nausea, GI discomfort, and changes in eating patterns can disrupt sleep, particularly in the first weeks. Sleep deprivation causes fatigue that's not related to weight loss itself.

Fatigue during the first 2–4 weeks on Mounjaro is common and usually self-limiting. The nausea settles, appetite adjusts, eating patterns stabilise. If fatigue persists beyond 4–6 weeks and is significant, blood tests are warranted — particularly ferritin, B12, and thyroid. These are the most commonly missed treatable causes.
