Taking 1,000mg of vitamin C in tablet form and having 1,000mg delivered by IV are not the same thing. The difference in plasma concentration is approximately 70-fold. Whether that difference matters depends entirely on what you're trying to achieve.
The Absorption Problem With Oral Vitamin C
The gut uses a sodium-dependent transporter (SVCT1) to absorb ascorbic acid. This transporter becomes saturated at around 200–400mg per dose. Beyond that, absorption efficiency falls sharply — doses above 1,000mg achieve less than 50% absorption, and much of the excess is excreted in urine. This is why high-dose oral supplementation causes GI upset and loose stools: the unabsorbed vitamin C reaches the colon intact.
The maximum plasma concentration achievable from oral supplementation — even with very large, frequent doses — is approximately 200–250 µmol/L.
What IV Delivery Changes
Because IV vitamin C bypasses gut absorption entirely, plasma concentrations 70x higher than oral intake are achievable — reaching 10,000–20,000 µmol/L with high-dose protocols. This isn't clinically relevant for general supplementation. But it becomes significant for specific functional outcomes:
- Immune activation — high-dose IV vitamin C supports neutrophil and natural killer cell function at plasma concentrations that oral intake can't reach
- Collagen synthesis — vitamin C is a required co-factor for collagen production; higher delivery accelerates tissue repair after injury or surgery
- Antioxidant load — at supraphysiological concentrations, ascorbate acts as a pro-oxidant in specific tissues, which is the basis for its oncological research interest
- Adrenal support — the adrenal glands contain the highest vitamin C concentration of any organ; acute depletion under stress is better replenished intravenously

Who Tends to Notice the Difference
Patients who report the clearest benefit from IV vitamin C delivery typically fall into a few categories: those recovering from illness (particularly viral illness or bacterial infections); patients preparing for or recovering from surgery; athletes with very high oxidative load from training; and people under significant physiological or psychological stress with recurrent infections or slow recovery. For healthy individuals who eat well, oral supplementation achieves adequate daily levels without requiring IV.
