Nebido is the most commonly prescribed testosterone in the UK — but it's not right for everyone. If you're experiencing peaks and troughs, poor symptom control in the weeks before your next injection, or mood instability, switching to a shorter-acting ester like enanthate could give you significantly better stability.
Why Some Men Do Better on Enanthate
Nebido (testosterone undecanoate) is a long-acting ester with a half-life of around 33–40 days. It's administered every 10–14 weeks. The upside is fewer injections. The downside is that many men feel great in weeks 1–4, acceptable in weeks 5–8, and noticeably symptomatic again by weeks 9–12 as levels trough. This "rollercoaster" effect is one of the most common reasons men seek a switch.
Testosterone enanthate has a half-life of 4–5 days. Injected every 1–2 weeks, it maintains far more stable blood levels — and when injected weekly, produces the flattest curve of any injectable TRT protocol.

Is a Switch Possible?
Yes — and it's straightforward. The main consideration is timing. Because Nebido has such a long half-life, there's no risk of testosterone levels dropping suddenly when you switch. Your GP will typically start enanthate 4–6 weeks before your next Nebido would have been due — effectively letting the Nebido taper out while the enanthate builds a steady state.
What the Switch Involves
- Consultation — review of current symptoms, last injection date, and recent testosterone levels
- Baseline bloods — testosterone (trough, ideally), haematocrit, PSA, LFTs
- Protocol set — your GP will determine starting dose and frequency based on your trough level
- Self-injection training — most patients inject into the thigh or glute; takes 60 seconds once learned
- 6-week review bloods — to confirm stable state levels and adjust dose if needed
Will I Need to Come Off Nebido Before Starting?
No. The switch is additive initially — your Nebido levels are still present when enanthate begins. Your GP will time the first enanthate injection to coincide with a point where Nebido is tapering, ensuring a smooth transition without low-testosterone symptoms.
