Why Am I Always Tired? 12 Blood Tests That Could Reveal the Cause

Chronic fatigue is rarely "just stress." If you've been exhausted for weeks or months despite adequate sleep, something medical is almost certainly going on β€” and a blood test can find it.

When Tired Isn't Normal

Everyone feels tired sometimes. But if you're waking up exhausted after 8 hours of sleep, dragging yourself through the afternoon, or relying on caffeine just to function β€” that's not normal tiredness. That's a symptom.

Persistent fatigue affects roughly one in five people in the UK at any given time, yet it remains one of the most underinvestigated complaints in general practice. The NHS often screens for anaemia and thyroid, returns a "normal" result, and leaves patients no better off. The problem isn't that tests don't exist β€” it's that not enough of them are being ordered.

Here are the 12 blood tests that give the most complete picture of why you might be constantly tired.

The 12 Key Blood Tests for Chronic Fatigue

1. Full Blood Count (FBC)

The starting point. FBC measures your red blood cells, haemoglobin, haematocrit, white blood cells, and platelets. Low haemoglobin signals anaemia β€” one of the most common causes of fatigue. It also flags signs of infection, inflammation, and bone marrow problems.

2. Ferritin (Iron Stores)

Ferritin is one of the most important tests for fatigue that's routinely missed. You can have a completely normal FBC and still have critically depleted iron stores. Ferritin below 30 mcg/L is associated with fatigue, brain fog, hair loss, and breathlessness β€” even when haemoglobin is normal. Many laboratories flag results as "normal" above 12 mcg/L, but symptoms often persist until ferritin exceeds 50–70 mcg/L.

3. Serum Iron & TIBC

Together with ferritin, these give a full picture of iron metabolism. Total Iron Binding Capacity (TIBC) rises when iron stores are depleted β€” the body tries to maximise absorption of whatever iron is available. This pattern (low ferritin + high TIBC) is a clear flag for iron deficiency even when FBC is normal.

4. TSH (Thyroid-Stimulating Hormone)

The first thyroid test. A raised TSH indicates your thyroid is underperforming (hypothyroidism), causing fatigue, weight gain, cold intolerance, and brain fog. TSH alone misses a lot β€” many symptomatic patients fall in the upper-normal range. But it's a necessary first step.

5. Free T4 & Free T3

If TSH is normal but you still feel terrible, checking Free T4 and Free T3 gives a more granular view. Some people don't convert T4 to active T3 efficiently β€” a problem not visible in TSH alone. Low-normal Free T3 with normal TSH is a common finding in people with treatment-resistant fatigue.

6. Vitamin D (25-Hydroxyvitamin D)

Vitamin D deficiency is endemic in the UK, where low sunlight limits natural production for most of the year. Below 50 nmol/L is considered deficient; below 75 nmol/L is suboptimal. Fatigue, muscle weakness, low mood, and frequent infections are all hallmarks of low vitamin D. Optimal levels for energy are typically 100–150 nmol/L.

7. Vitamin B12

B12 is essential for nerve function and red blood cell formation. Deficiency causes fatigue, tingling, poor memory, and mood changes. Strict vegetarians and vegans, people on long-term metformin or PPIs, and anyone over 60 are at elevated risk. Many labs report results as "normal" above 200 pg/mL, but many clinicians consider levels below 400–500 pg/mL suboptimal for energy.

8. Folate

Works alongside B12 in red blood cell production and DNA synthesis. Folate deficiency causes megaloblastic anaemia and neurological fatigue. Often tested alongside B12 as deficiency in one affects interpretation of the other.

9. Cortisol (Morning)

Cortisol is your primary stress hormone and energy regulator. Low morning cortisol β€” often called adrenal fatigue or adrenal insufficiency depending on severity β€” causes profound, often debilitating exhaustion that's worst in the morning. Chronic stress depletes cortisol output over time. This is one of the most commonly overlooked causes of fatigue in high-achieving adults.

10. HbA1c & Fasting Glucose

Blood sugar dysregulation β€” from prediabetes to full type 2 diabetes β€” is a major fatigue driver. HbA1c reflects average glucose over three months. Even borderline elevations (41–47 mmol/mol) are associated with significant energy crashes, brain fog, and afternoon slumps. Fasting glucose adds a same-day snapshot.

11. Testosterone (Men) / Oestrogen & Progesterone (Women)

Hormonal deficiency is a major and frequently overlooked cause of fatigue. Low testosterone in men causes exhaustion, low motivation, and poor sleep quality. In women, perimenopause and menopause β€” which can begin in the early 40s β€” cause profound fatigue driven by declining oestrogen and progesterone. These tests are rarely ordered in standard NHS fatigue workups.

12. C-Reactive Protein (CRP) & ESR

Inflammatory markers. Chronic low-grade inflammation β€” from autoimmune conditions, undiagnosed infection, gut dysfunction, or even visceral fat β€” is a well-established cause of fatigue. Elevated CRP without an obvious source warrants deeper investigation. ESR is a more sensitive (though less specific) marker of systemic inflammation.

Why "Normal" Results Don't Always Mean You're Fine

This is the most frustrating part of fatigue investigation. You get your results back, your GP says everything is "normal," but you feel no better. The issue is how reference ranges are set: they reflect the middle 95% of the general population β€” which includes many people who are also exhausted.

Optimising, not just normalising, is the difference between feeling acceptable and feeling well. A ferritin of 15, a vitamin D of 48, a Free T3 at the bottom of the range β€” all technically "normal," all associated with significant fatigue at a clinical level.
