What Does a Ferritin of 12 Actually Mean?
Ferritin is the protein that stores iron in your body. When a blood test returns a ferritin of 12 µg/L, it means your iron stores are at or below the lower end of the conventional reference range (typically 12–150 µg/L for adults, though ranges vary slightly by laboratory).
Here's the problem with "it's within range": the bottom of that range represents barely enough to survive, not optimal health. Most iron specialists and GP guidelines consider a ferritin below 30 µg/L to be clinically significant iron deficiency — even if it hasn't yet caused anaemia. A ferritin of 12 is not just borderline. It is genuinely depleted.
Why "within range" can be misleading: Laboratory reference ranges are calculated from population data — they include the lowest 2.5% as the cutoff. A ferritin of 12 is statistically "normal" but clinically consistent with symptomatic iron deficiency. The functional threshold that most guidelines use is 30 µg/L for absolute deficiency.
Symptoms Associated With Ferritin 12
At a ferritin of 12, many patients experience a recognisable cluster of symptoms — often dismissed or attributed to stress, poor sleep, or being "just tired":

If your ferritin is 12 and you're experiencing several of these symptoms, it is very likely that the iron deficiency is contributing — regardless of whether your haemoglobin is technically "normal".
Can You Have Iron Deficiency With Normal Haemoglobin?
Yes — and this is one of the most commonly missed clinical diagnoses. The medical term is iron deficiency without anaemia (IDWA), and it's increasingly recognised as a significant condition in its own right.
The body has a well-designed hierarchy for using available iron. It prioritises keeping haemoglobin normal above all else, because haemoglobin is needed to carry oxygen to vital organs. As iron stores drop, the body draws on them to maintain Hb — meaning Hb can stay in the normal range while ferritin falls to very low levels. By the time Hb drops, ferritin may have been depleted for months.
So a ferritin of 12 with normal Hb is not reassuring — it means the body has been compensating, and the compensation is nearly exhausted.
Iron Tablets vs Infusion at Ferritin 12: Which Is Better?
With a ferritin this low, you have two main options: oral supplementation or IV iron infusion. Both can work — but they are not equivalent in every situation.

- You've already tried oral iron without significant improvement

- Your symptoms are significantly affecting quality of life and you need faster relief
- Your TSAT is also below 20%, indicating functional deficiency on top of depleted stores
How Much Iron Will You Need at Ferritin 12?
The answer depends on your haemoglobin level as well as your ferritin. If your Hb is still within normal range, the iron deficit is primarily about restoring stores. If Hb is also low, there is an additional requirement to raise it to target (15.0 g/dL in the Ganzoni Formula used clinically).
